Calcium Folinate

Calcium folinate is an active derivative of folic acid, primarily used to aid in the detoxification of chemotherapy drugs (such as methotrexate) or to treat megaloblastic anemia caused by folic acid deficiency. It bypasses folic acid metabolism barriers, directly participating in DNA synthesis, reducing drug toxicity, or correcting nutritional deficiencies.

Mechanism of Action

• Bypassing Metabolic Barriers: Calcium folinate can be directly converted to tetrahydrofolate without the catalysis of dihydrofolate reductase, helping cells repair DNA.
• Detoxification Effect: When used in combination with methotrexate, it can reduce the latter's damage to normal cells and protect bone marrow and mucous membrane tissues.

Pharmacological Mechanism of Action

• Active Folic Acid Form: Calcium folinate is the activated form of folic acid in the body, directly participating in the synthesis of DNA, RNA, and proteins without requiring conversion by dihydrofolate reductase.
• Competitive Protective Mechanism: As an antidote, it competitively binds to dihydrofolate reductase, causing the dissociation of methotrexate bound to the enzyme, restoring intracellular tetrahydrofolate levels, thereby reversing the damage of methotrexate to bone marrow and gastrointestinal mucosa.
• Stabilizing Enzyme Complex: As an adjuvant chemotherapy drug, it stabilizes the binding of fluorouracil to thymidine synthase, inhibiting DNA synthesis in cancer cells and enhancing the killing effect of chemotherapy drugs on tumor cells.
