Saw Palmetto Extract is derived from the mature, dried fruit of *Serenoa repens* (a plant in the Arecaceae family). The fruit is rich in constituents such as free fatty acids and phytosterols. Saw palmetto extract has the following properties:

The mechanism of male pattern baldness is that testosterone and type II 5α-reductase in the body metabolize to produce DHT. Hair follicles are susceptible to DHT attack, resulting in atrophy and hair loss.

A double-blind, randomized controlled trial (RCT) enrolled 124 patients with severe BPH (International Prostate Symptom Score [IPSS] > 19); patients were randomized to receive either saw palmetto fruit extract (320 mg/day) or tamsulosin (0.4 mg/day).

Results showed that after 12 months of treatment, the IPSS decreased by 7.8 points in the saw palmetto group and by 5.8 points in the tamsulosin group (P = 0.051), indicating comparable efficacy between the two groups. Another double-blind RCT demonstrated that, in the treatment of moderate BPH over six months, the reduction in IPSS with saw palmetto fruit extract was comparable to that observed with finasteride.

Regarding sexual function scores, saw palmetto fruit extract outperformed finasteride, with fewer reported complaints of decreased libido and erectile dysfunction.

A single-center, randomized controlled study involving 350 patients with CP/CPPS (aged 28–40 years) demonstrated that three months of treatment with saw palmetto fruit extract effectively improved mean total sperm motility and reduced total NIH-CPSI and IPSS scores; outcomes were superior when combined with tadalafil and levocarnitine.

A randomized, double-blind, placebo-controlled study evaluated the efficacy of oral saw palmetto fruit extract in treating hair loss among adult men and women who perceived themselves as having thinning hair.

After 90 days of treatment, subjects in the active treatment group showed significant increases in terminal hair, vellus hair, and total hair counts in both the frontal and vertex regions of the scalp; compared to the placebo group, the active treatment group demonstrated a sevenfold greater improvement in terminal hair count and a twelvefold greater improvement in total hair count. Hair pull tests revealed a significant reduction in the total number of hairs lost. No treatment-related adverse events occurred during the study, indicating that daily use of saw palmetto fruit extract is a safe and effective method for promoting hair growth.
