 Pellets are a multi-unit oral dosage form; a single dose typically consists of dozens to hundreds of small spherical units. They can be formulated into various types, such as sustained-release, controlled-release, or enteric-coated pellets.


Matrix-type sustained-release preparations
Matrix-type sustained-release preparations consist of a matrix formed from one or more polymeric materials that carries the active pharmaceutical ingredient; this composite is then processed into various solid dosage forms. Depending on the matrix material used, these preparations can be further classified into hydrophilic gel matrix systems, insoluble matrix systems, and erodible matrix systems.


Pellet-based sustained-release preparations
Pellets are spherical or quasi-spherical dosage forms with diameters ranging from 0.5 to 1.5 mm. Sustained-release pellets are composed of a core, a drug layer, a sustained-release layer, and a protective layer; the functions of these components are illustrated in the accompanying figure. The excipients used in sustained-release pellets determine the drug release rate and the site of release; selecting appropriate excipients enables the production of pellets with uniform particle size, smooth surfaces, and stable release profiles. Variations in the type, quantity, and method of adding excipients allow for the creation of pellets with different performance characteristics. While there is extensive research on Traditional Chinese Medicine (TCM) pellets-focusing primarily on preparation methods, excipient selection, and *in vitro* release-there is relatively limited research on the relationship between the properties of TCM raw materials (such as viscosity, flowability, and hygroscopicity) and pellet-forming characteristics, as well as on the *in vivo* absorption profiles of these pellets. Strengthening research in these areas is essential to advance the development of TCM sustained-release pellet formulations.
Compared to conventional oral dosage forms, sustained-release pellets offer advantages such as excellent flowability, high bioavailability, and well-defined drug release characteristics, making them a subject of increasing interest among researchers.

Comparison of Pellet Preparation Processes

 

Extrusion-Spheronization Method
Nakahara first invented the extrusion-spheronization pelletization technique in 1964, while Reynolds, Conine, and others pioneered the comprehensive introduction and application of this method in the field of pharmaceutics. It is currently one of the most widely used techniques forSustained Release Pellets pellet preparation. The basic steps involve adding a binder to the drug and excipient powders and mixing them thoroughly; extruding the mixture into cylindrical strands using an extruder; cutting and rolling these strands into uniform spherical pellets in a spheronizer; and finally, drying the product. The process can be summarized as mixing, wet massing, extrusion, spheronization, and drying.

Hot-Melt Extrusion Method
Hot-melt extrusion primarily involves three steps: hot-melt softening, shaping, and solidification. The required materials are fed into a barrel with zoned temperature control and propelled forward by a screw. After melting or softening in specific zones, the materials are uniformly mixed through the action of shear and mixing elements and finally extruded through a die at a controlled pressure, speed, and shape. During extrusion, the particle size of the multi-component materials decreases while they undergo spatial rearrangement and interpenetration, ultimately achieving mixing at the molecular level and transitioning from a multiphase state at the inlet to a single-phase state at the outlet. The extrudate is then placed in a spheronizer and spheronized under heated conditions to yield the pellets. The key difference between hot-melt extrusion and extrusion-spheronization is that the latter relies on a binder to ensure material plasticity, whereas the former relies on heat.

Congealing Granulation Method
Congealing granulation is a novel, simple process suitable for preparing matrix-type pellets. First, the carrier material is melted at a temperature 5–10°C above its melting point, and the drug is thoroughly mixed with the carrier to form a solution or suspension. This mixture is then sprayed through a nozzle into an inert liquid, where it solidifies to form pellets. This process consists of two stages: forming and solidification. The former operates at 25–100°C, involving heating and melting, while the latter operates at 0–40°C, involving cooling and solidification. Depending on the density difference between the molten mixture and the inert liquid, the melt can be injected into the inert liquid from either the top or the bottom, ensuring the pellets are sufficiently cooled as they sink or float. Pellets produced via this method exhibit good sphericity and a narrow particle size distribution.

Freeze-drying granulation method
A drug-containing solution, emulsion, or suspension is rapidly sprayed into liquid nitrogen at -160°C to induce immediate solidification; water or organic solvents are subsequently removed via freeze-drying to yield the final product. This method can be used to produce both immediate-release and sustained-release pellets. Excipients in immediate-release pellet formulations typically include fillers (such as lactose and mannitol) and binders (such as polyvinylpyrrolidone); sustained-release formulations require the addition of release-retarding materials in addition to fillers and binders. The advantages of the freeze-drying granulation method include its suitability for heat- and moisture-sensitive drugs, as well as the production of pellets that are uniform, spherical, and possess a porous structure.
